Data Request
To request data eraser and or other individual data-related requests, contact info@getmtraining.online with a formal request that follows the outline below. You must include the following information or your request will not be able to be processed:

    3. Your account email (if contacting us from a seperate email then your account email)
    
    4. Submitting request as?
        Option 1: "The individual, or the legal parent/guardian of the individual whose name is listed under "first name and last name"
        Option 2: "An authorized agent granted permission by the consumer to make this request on their behalf"
    
    5. Under what rites are you making this request?
        option 1: "GDPR"
        option 2: "CCPA"
        option 3: "other"
    
    6. I am submitting a request to
        option 1: have a data eraser of my information
        option 2: opt out of my data being sold to third parties
        option 3: opt into the sale of my personal data
        option 4: other (please specify)
    
    7. Specification of number 6 (if any) and any additional comments.

I understand that I will be contacted in regards to my request, and that I will be required to validate my request via email.

I understand in the process of a data eraser, I will permenatly lose all data and that the action is ireversible and can result in termination of services, packages, and any other interactions I have had with the GETM community. That can result in loss of session credits as well as paid services that are unclaimed.

I verify that all information listed above is accurate and true, and that my failure to provide accurate and truthful information will result in failure of request completion due to inadequete information provided, and/or penalty of perjury."
